DO YOU HAVE ALL THE IMPORTANT DOCUMENTS FOR YOUR APPOINTMENT WITH US? YOU CAN CHECK THIS WITH OUR PRACTICAL CHECKLIST:


Before your first appointment, please fill out our medical history form and bring it with you when you visit us. If you are unable to print it out at home, we will have a copy ready for you at our practice.

Checklist

  • electronic health card (eGK) if you have statutory health insurance

  • Private insurance card and identity card, if applicable

  • If you regularly take medication: a list of medications

  • For allergies: an allergy passport (if available)

  • For heart disease: a heart passport (if available)

  • If you have already been fitted with dental implants: Your implant passport (if available)

  • Current x-rays (if available, not older than 1 year)

  • Preliminary findings (if available)

  • Bank transfer (if available)

Important information about your appointments

We operate our practice on an appointment-only basis. This means that every appointment is reserved exclusively for you. This allows us to guarantee that we can focus entirely on your treatment and that you will not experience any unnecessary waiting times.

If you are unable to attend an appointment, please cancel at least 24 hours in advance . This gives us the opportunity to offer the appointment to another patient in pain.

Please understand that in the event of unexcused non-attendance or very short-notice cancellations, we will have to charge a cancellation fee in accordance with § 615 BGB (German Civil Code), unless we are able to reallocate the time.
